Hints for Entering Search Criteria

In order to narrow your search for physicians, you can specify up to 5 different search criteria - Last Name, Hospital, Managed Care Plan, Specialty and Zip Code.

Last Name:

Enter the last name of the physician, if you know it. You may use the % as a wildcard, e.g. smi%.

Hospital:

Select a hospital from the list to limit the search to physicians affiliated with that hospital.

Managed Care Plan:

Select a Managed Care Plan from the list to limit the search to physicians participating in that Plan. You can expect the search to take longer if you only specify a Plan because of the volume of Plan data that must be reviewed.

Specialty:

Select a Specialty from the list to limit the search to a particular area of specialization.

Zip Code/County:

Enter/Select a Zip Code or County to limit the search to physicians with offices in specific areas. A physician record will be returned if Any of his/her addresses meet the location criteria.

Reset:

Press the Reset button to clear out the search criteria.

Search:

Press the Search button to submit the search criteria and return search results.
